Moreira et al reported prognostic factors for a patient with infective endocarditis. These can help to identify a patient who may require more aggressive management. The authors are from Hospital de Santa Marta and Centro Hospitalar de Lisboa Central in Lisbon.
Patient selection: adult with infective endocarditis using the modified Duke criteria
Predictors of mortality:
(1) COPD (OR 4.9)
(2) cerebral embolism (OR 9.4)
(3) endocarditis due to a Staphylococcus species (OR 3.8)
(4) endocarditis due to a non-HACEK Gram-negative bacteria (OR 12.9)
(5) failure to perform surgery when there were surgical indications
Indications for surgery:
(1) heart failure
(2) uncontrolled infection
(3) serious risk of emboli
Rapid identification of patients at greater risk and aggressive management may improve outcomes.
