Description

Lopez-Penebad et al identified prognostic factors for a patient with a Hurthle cell neoplasm of the thyroid. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Texas M.D. Anderso Cancer Center in Houston.


 

Patient selection: Hurthle cell neoplasm

 

Risk factors for worse survival:

(1) multifocal disease

(2) lymph node metastases (associated with distant metastases)

(3) distant metastases

(4) extraglandular invasion

(5) tumor size

(6) older age

(7) foci of anaplastic thyroid carcinoma

 

where:

• The mean age for Hurthle cell carcinoma was 51.8 years.

• The mean diameter of a Hurthle cell carcinoma was 4.3 cm.

 

Risk factors for improved survival:

(1) tumor encapsulation

(2) (absence of residual disease = complete surgical excision) AND (radioactive thyroid therapy)

(3) Hurthle cell adenoma
