Description

Grani et al identified prognostic factors for a patient with suspected myocarditis, including the appearance of the heart of cardiac magnetic resonance imaging. These can help to identify a patient who may benefit from more aggressive management. The authors are from Brigham adn Women's Hospital and the University of Michigan.


Patient selection: suspected myocarditis

 

Outcome: major adverse cardiovascular events (MACE; death, heart failure, heart transplant, sustained ventricular arrhythmia, recurrent myocarditis)

 

Prognostic factors associated with a worse outcome:

(1) left ventricular ejection fraction < 40%

(2) low right ventricular ejection fraction

(3) late gadolinium enhancement (LGE)

(3a) extensive LGE

(3b) patchy or infarct-like distribution of LGE

(3c) LGE of the septum and midwall
