Description

Fox et al reported prognostic factors for women with adult granulosa cell tumors of the ovary. These can help identify a woman who may benefit from closer monitoring and more aggressive management. The authors are from the University of Manchester and St. Mary's Hospital in Manchester, England.

Factors associated with a poor prognosis:

(1) age of the patient > 40 years at the time of diagnosis

(2) presentation due to abdominal symptoms

(3) presence of a palpable mass

(4) diameter of the tumor > 15 cm

(5) extra-ovarian spread

(6) bilateral ovarian tumors

(7) solid growth throughout

(8) presence of numerous mitotic figures

The opinion of the authors is that all adult granulosa cell tumors are potentially malignant. All patients with these tumors should be followed for spread or recurrence, especially any of these factors are present.
