Description

Fiore et al evaluated myxoid, round cell and pleomorphic liposarcomas for factors affecting mortality. These can help identify a patient who may benefit from aggressive or novel therapy. The authors are from the Italian National Cancer Institute in Milan.


 

Types of liposarcoma evaluated:

(1) myxoid

(2) round cell (round cells comprise > 5% of cellularity)

(3) pleomorphic

 

Parameters affecting disease specific survival:

(1) presentation (initial vs recurrent)

(2) tumor diameter in cm

(3) histologic grade

(4) surgical margins at resection

where:

• Location in an extremity and the use of radiation therapy impact local control and therefore likelihood of recurrent disease (local recurrence free survival).

• I would assume that tumor diameter and surgical margins refer to the initial resection.

Interpretation:

• minimum number of poor prognostic factors: 0

• maximum number of poor prognostic factors: 4

 

Additional factors that impact metastases and post-metastases survival:

(1) histologic grade (metastases more likely with Grade II or III)

(2) histologic type (myxoid better than round cell or pleomorphic)

(3) extrapulmonary metastases
