Description

Endorff and Gamelli evaluated patients with inhalation injury associated with thermal burns. The authors are from Loyola University in Maywood, Illinois.


Patient selection: thermal burns

 

Parameters:

(1) inhalation injury using the grades of Albright et al

(2) PaO2 to FIO2 ratio

 

The presence of significant inhalation injury (Grades 2 to 4, corresponding to moderate to massive injury) is associated with a worse outcome (with survival of 57% vs 84% for Grades 0 or 1).

 

A PaO2 to FIO2 ratio < 350 was associated with a higher fluid resuscitation requirement (7.4 mL per kg per percent TBSA; compared to 5.9 if ratio > 350). Survival was worse but did not meet statistical significance.
