Description

Akkaya et al identified prognostic factors for a woman with adenocarcinoma of the fallopian tube. These can help to identify a patient who may require more aggressive management. The authors are from Tepecik Obstetrics and Gynaecology Training and Research Hospital and Erzincan University in Turkey.

Patient selection: adenocarcinoma of the fallopian tube

Factors associated with a worse prognosis from multivariate analysis:

(1) stage III or IV (HR 3.6)

(2) ascites (HR 2.1)

(3) residual tumor size after surgery > 1 cm (HR 3.2), indicative of suboptimal surgery

where:

• Residual volume may be easier to estimate than diameter, especially if there are multiple small lesions. A sphere 1 cm in diameter has a volume of 0.52 cubic cm.

Additional factors associated with a worse prognosis:

(1) preoperative serum CA-125 >= 35 U/mL

(2) nonserous histology

(3) higher histologic grade (poorly differentiated)
