Baylisascaris larva migrans is difficult to treat once the central nervous system becomes involved. It is often progressive with a poor response to antihelminthic therapy.
Poor prognostic factors:
(1) moderate to severe neurologic impairment
(2) heavy exposure to eggs
(3) failure to treat with albendazole and corticosteroids within 1 week of exposure
(4) marked CSF eosinophilic pleocytosis (due to release of eosinophil-derived neurotoxin)
