Description

Suh et al used the product of serum alpha-fetoprotein (AFP and prothrombin induced by vitamin K absence-II (PIVKA-II) as a predictor of aggressive behavior by a hepatocellular carcinoma. This can help to identify a patient who may benefit from more aggressive management. The authors are from Seoul National University and Seoul National University Boramae Medical Center in Korea.


Patient selection: single hepatocellular carcinoma < 3 cm in diameter managed by surgical resection or radiofrequency ablation

 

Parameters:

(1) serum alpha-fetoprotein (AFP) in ng/mL (method and reference range not reported)

(2) serum PIVKA-II (units, method and reference range not reported; units may be mAU/mL based on report of Kang et al)

 

product of AFP and PIVKA-II =

= (serum AFP) * (serum PIVKA)

 

Interpretation:

• A product >= 1600 had early tumor recurrence and poor late survival.
