Description

A patient who is the receptive partner of anal intercourse may develop proctitis caused by lymphogranuloma venereum (LGV). This may mimic inflammatory bowel disease (IBD).


 

Clinical features:

(1) proctitis that may mimic inflammatory bowel disease in symptoms and endoscopic appearance

(2) mucosal ulcerations may be present

(3) chronic infections may be complicated by scarring and fistulae

 

Concurrent infections may include:

(1) HIV disease

(2) other sexually transmitted disease

(3) hepatitis B or C

 

Laboratory and pathologic findings:

(1) antibody to C. trachomatis

(2) positive polymerase chain reaction (PCR). False negative readings may occur if there are inhibitory substances in the stool.

(3) A rectal biopsy may show cryptitis, crypt abscesses and granulomas.
