Description

Christiaens et al listed a number of blind spots that can occur in discharge planning and aftercare for a patient who has been admitted to a burn center. The presence of a blind spot can lead to suboptimal care for the patient. The authors are from the Belgian Healthcare Knowledge Centre in Brussels.


Patient selection: burn center

 

Blind spots in discharge planning and aftercare:

(1) lack of a written discharge protocol

(2) lack of an adequate patient-centered care plan

(3) care plan not accessible to all caregivers

(4) lack of adequate training in specialized care for rehabilitation and home care providers

(5) contradictory information and instructions

(6) failure to adequately educate the patient about long-term needs and what to expect

(7) inadequate measures of quality in care delivery

(8) failure to follow burn care guidelines

(9) failure to adequately address problems faced by the patient (disability, social support, psychological, economic, disfigurement)
