The nasal turbinates may become swollen in a patient with sickle cell anemia.
Patient selection: sickle cell anemia
Mechanism: vascular occlusion and engorgement, mimicking priapism of the penis
Clinical features:
(1) nasal obstruction
(2) nasal pain
(3) turbinates swollen on exam, blocking air flow
(4) exclusion of other diagnoses
The condition is made worse by the use of topical vasoconstrictors.
Management should focus on correcting any condition that may have precipitated the event (dehydration, infection, etc) plus supplemental oxygen.
The turbinates can be resected if the condition is a significant problem for the patient.
