A patient with chronic myelogenous leukemia (CML) may develop priapism.
Mechanisms may include:
(1) leukostasis associated with hyperleukocytosis, primarily in the corpus cavernosa and dorsal veins of the penis
(2) arterial emboli and/or venous thrombi
(3) massive splenomegaly with compression of intra-abdominal veins
Clinical features:
(1) Priapism may be a presenting sign of underlying CML.
(2) hypersplenism
(3) systemic manifestations (fatigue, weight loss, asthenia)
Complications:
(1) erectile dysfunction, especially if there is a delay in presentation
