Description

Different medications may look similar to a person who is in a hurry or who is making assumptions. Errors in "look-alikes" can be eliminated if certain precautions are taken.


 

Look-alikes:

(1) similar but different names

(2) similar but different color codes

(3) similar shapes (container, capsule, pills)

(4) same drug, different strengths

(5) same drug, different formulations

 

Never:

(1) rely on color coding alone

(2) rely on storage location alone

(3) rely on drug name alone

 

Always:

(1) carefully read the product label for product name, strength and formulation when selecting a drug from inventory

(2) carefully review the drug name and dose on the product label before dispensing or administering any drug

(3) use color photographs or other visual aids to help distinguish similar products

(4) orient new employees to proper procedure to reduce mix-ups

 

Regularly:

(1) check inventories for possible stock mix-ups

(2) check inventory locations to make sure similar products are kept away from each other

(3) educate staff about products that may pose a problem
