Description

Gutierrez et al reported the PREPS (Prehospital Preparation for Surgery) score to identify an adult trauma patient who requires an early laparotomy. This partially depends on prehospital data. The authors are from University of Southern California in Los Angeles.

Patient selection: adult trauma

Outcome: laparotomy within 2 hours of arrival at the trauma center

Parameter:

(1) sex

(2) blunt mechanism

(3) penetrating mechanism

(4) Glasgow Coma Scale (GCS) from 3 to 15

(5) systolic blood pressure in mm Hg

(6) chest wall deformity or instability

(7) pelvic fracture

Interpretation:

• minimum score: 0

• maximum score: 20

• A cut-off value was not seen in the paper.

Performance:

• The area under the ROC curve is 0.79 in the derivation and 0.78 in the validation cohorts.
