Criteria for cryoglobulinemic vasculitis:
(1) The patient is positive for cryoglobulins on at least 2 occasions at least 12 weeks apart.
(2) The patient has at least 2 of the 3 “questionnaire” items.
(3) The patient has at least 3 out of 4 clinical items (past or present).
(4) The patient has at least 2 out of 3 laboratory findings.
Questions – Have you?
(1) had one or more episodes of small red spots on the skin (especially legs)?
(2) had red spots on the lower extremities which become brownish during resolution?
(3) been told that you have viral hepatitis by a doctor ?
Clinical items:
(1) constitutional symptoms (fatigue, low-grade fever, unexplained fever, fibromyalgia)
(2) articular involvement (arthralgia and/or arthritis)
(3) vascular involvement (purpura, skin ulcers, necrotizing vasculitis, hyperviscosity, Raynaud’s)
(4) neurological involvement (peripheral neuropathy, cranial nerve palsy, CNS vasculitis)
Laboratory findings:
(1) reduced serum C4
(2) positive serum rheumatoid factor
(3) positive serum M (monoclonal) component
