Wright et al identified factors associated with mortality in a patient with meliodosis. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Washington Seattle, Mahidol University (Bangkok), University of Pittsburgh, and multiple hospitals in Thailand.
Patient selection: meliodosis
Predictors of mortality:
(1) female sex (odds ratio 2.5)
(2) elevated modified SOFA score (use of vasopressors without doses; substitution of ventilatory support for arterial blood gases)
(3) number of organ failure(s)
(4) elevated serum IL-6 and/or IL-8
The risk of mortality increases as the number of predictors increase.
Other cytokines were also associated with elevated 28-day mortality (C-CSF, interferon, TNF-alpha, IL-1, IL-10). Measurement of cytokines may be limited in resource-limited settings.
