Description

Wang et al identified risk factors for one-year mortality for a patient discharged following an episode of Acute Respiratory Distress Syndrome (ARDS). These can help to identify a patient who may benefit from more aggressive management. The authors are from Taichung General Hospital (Taiwan), University of California San Francisco, and Vanderbilt University.


Patient selection: discharged from hospital after an episode of ARDS (survivor)

 

Outcome: 1-year mortality

 

Predictors of 1-year mortality:

(1) older age (odds ratio 1.36 per decade)

(2) discharge location (see below)

(3) malignancy, especially if metastatic

(4) HIV

(5) chronic kidney disease

(6) prolonged hospitalization (odds ratio 1.03 per day' odds ratio 1.9 at 21 days)

 

Factors associated with reduced risk:

(1) living at home prior to admission

(2) trauma as cause for ARDS

 

Discharge location:

(1) Discharge to home or for rehabilitation is associated with a good outcome.

(2) Discharge to a nursing home or another hospital was associated with a worse outcome.

(3) Discharge to hospice was associated with short-term mortality.
