Description

Walter et al identified a number of risk factors for pneumonia following an acute stroke (stroke-associated pneumonia). These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Rostock in Germany.


 

Patient selection: acute stroke

 

Outcome: acute pneumonia

 

Early onset pneumonia was defined as onset within 72 hours of admission.

 

Risk factors for acute pneumonia:

(1) dysphagia (relative risk 10)

(2) National Institute of Health Stroke Scale (NIHSS) >= 10 within 24 hours of admission (relative risk 6.6)

(3) non-lacunar basal ganglion infarction (relative risk 3.1)

(4) infection other than pneumonia present on admission (relative risk 3.8)

 

Performance:

• The NIHSS >= 10 had a sensitivity of 82% and specificity of 71%.

• The presence of dysphagia had a sensitivity of 80% and specificity of 70%.
