Vidal et al reported risk factors for postoperative pneumonia in a patient who has had a heart transplant. The authors are from Sorbonne University and Centra HospitalierUniversitaireFleix Guyon in France.
Patient selection: status post heart transplantation
Most common causes of postoperative pneumonia:
(1) Enterobacteriaceae
(2) Pseudomonas aeruginosa
Risk factors:
(1) preoperative mechanical ventilation (OR 1.4)
(2) perioperative blood transfusion (OR 1.4)
Other factors that may contribute:
(1) diabetes mellitus
(2) revision surgery
(3) higher SAP2 score in the ICU
Postoperative pneumonia is associated with:
(1) 30-day mortality
(2) longer postoperative mechanical ventilation
(3) longer time to weaning from VA-ECMO
(4) longer length of stay in the ICU
