Description

Tanimura et al screened pregnant women at high risk for cytomegalovirus (CMV) infections. The presence of positive evidence for a CMV infection may help to decide on whether to perform amniocentesis in order to collect amniotic fluid for confirmatory testing. The authors are from Kobe University and Aisenkai Nichinan Hospital in Japan.


The authors advocated universal screening of pregnant women for IgM and IgG antibodies to CMV.

 

Patient selection: pregnant woman who is IgM positive against CMV antigens

 

Predictors of congenital CMV infection:

(1) positive PCR results in secretions of the uterine cervix (odds ratio 16)

(2) fetal abnormalities on ultrasonography suggestive of CMV infection (odds ratio 32)

 

Fetal findings on ultrasonography suggestive of CMV infection:

(1) fetal growth retardation

(2) microcephaly

(3) ventriculomegaly

(4) intracranial calcifications

(5) pleural effusion

(6) ascites

(7) hepatosplenomegaly

(8) hyperechoic bowel
