Description

Spiegelman et al identified predictors for blood transfusion in a woman undergoing a "high-order" cesarean delivery. This can help to idenitfy a patient who may benefit from more aggressive management. The authors are from Mount Sinai Hospital and Columbia University in New York City.


Patient selection: third or later Cesarean section ("high order", 2 or more previous to current delivery))

 

Predictors for blood transfusion:

(1) placenta praevia (associated with the highest risk)

(2) prophylactic anticoagulation during pregnancy

(3) labor prior to cesarean

(4) blood loss > 1,000 mL

 

In the absence of these predictors the rate of transfusion was very low.
