Sadler et al reported predictors of mortality for patients undergoing colectomy for ischemic colitis. These can help to identify a patient who may require more aggressive management. The authors are from the University of Calgary in Alberta, Canada.
Patient selection: colectomy for ischemic colitis, age 18 to 84 years
Outcome: mortality
Mortality related to age:
(1) 65 to 84 years: OR 5.5
(2) 50 to 64 years: OR 3.2
(3) 35 to 49: OR 1.8
Additional predictors of mortality:
(1) health insurance Medicare (OR 1.3) or Medicaid (OR 1.7) or self-pay (OR 1.3)
(2) liver disease (mild OR 3.5; moderate to severe 2.2)
(3) renal disease (OR 2.2)
(4) cancer (OR 1.7)
(5) AIDS/HIV (OR 2.4)
(6) congestive heart failure (OR 1.3)
(7) cerebrovascular disease (OR 1.5)
(8) peptic ulcer disease (OR 1.5)
Factors protective of survival:
(1) undergoing colonoscopy or sigmoidoscopy (OR 0.78 if either done)
(2) diabetes with chronic complications (OR 0.57)
