Roger et al reported predictors for stage II chronic kidney disease (CKD) in a patient with homozygous sickle cell disease. The authors are from the University of Lille, Assistance Publique-Hopitaux de Paris, and Sorbonne University.
Patient selection: homozygous sickle cell disease
Predictors of state II CKD from multivariate analysis:
(1) macroalbuminuria (HR 3.3)
(2) diastolic blood pressure > 70 mm Hg (HR 2.1)
(3) serum LDH (HR 1.3 per 100 U/L increase, presumably from upper limit if normal)
(4) age in years (HR 1.05 per year in text; 1.13 per year in abstract). For a patient over 30 years of age the HR is 13.3. A hazard ratio comparable to the other predictors can be seen in the 20's.
Additional predictors from multivariate analysis:
(1) tricuspid regurgitation velocity > 2.5 m/s (HR 2.9)
If a patient has one or more risk factors, then renal function should be monitored.
