Description

Ripoll et al identified factors associated with decompensation in a patient with compensated cirrhosis. These can help to identify a patient who may benefit from more aggressive management. The authors are from Veterans Affairs CT Healthcare System (West Haven), Yale University, Brigham and Women's Hospital, Faulkner Hospital, Royal Free Hospital, Hospital Germans Trias and Hospital Clinical Provincial de Barcelona.


Patient selection: compensated cirrhosis

 

Predictors for decompensation:

(1) hepatic venous pressure gradient (HVPG) > 10 mm Hg (unadjusted HR 5.7)

(2) MELD score >= 10 (unadjusted HR 2.3)

(3) serum albumin < 4.0 g/dL (unadjusted HR 2.0)

 

In addition, INR was a risk factor in univariate analysis (HR 14.3)

 

A patient with none of these factors is at low risk for decompensation.
