A patient with ulcerative colitis and an ileal pouch anal-anastomosis (IPAA) may develop a chronic pouchitis. A number of factors may identify a patient who is at increased risk for this complication.
Factors associated with pouchitis:
(1) pancolitis (with colitis extending to the cecum)
(2) appendiceal ulcerations
(3) fissuring ulcers
(4) diffuse chronic dudodenitis
A patient with one or more of these findings should be managed moer aggressively.
