Description

Ninh et al al reported predictors for sepsis following appendectomy in an adult. The authors are from the Icahn School of Medicine at Mount Sinai in New York City.


Patient selection: adult status post appendectomy

 

Occurrence: 0.43% out of 72,000 patients

 

Predictors of sepsis with adjusted odds ratio >= 2:

(1) African American race (adjusted odds ratio 2.0)

(2) acute renal failure or dialysis (adjusted odds ratio 4.6)

(3) disseminated malignancy (adjusted odds ratio 4.1)

(4) infection of the incision (adjusted odds ratio 4.3)

(5) dirty or infected incision (adjusted odds ratio 3.8)

(6) open appendectomy (adjusted odds ratio 2.6)

 

where:

Items 4 to 6 suggest more severe appendicitis, such as rupture.

 

Predictors of sepsis with adjusted OR from 1.5 to 1.9:

(1) age >= 60 years (adjusted OR 1.5)

(2) morbid obesity (adjusted odds ratio 1.8)

 

Factors associated with reduced risk of sepsis"

(1) female (adjusted odds ratio 0.69; alternatively. the OR is 1.4 for males)
