Description

Nangit et al identified factors associated with early hospital readmission for a patient with systemic lupus erythematosus. These can help to identify a patient who may benefit from more aggressive management. The authors are from Cedars-Sinai Medical Center in Los Angeles.


Patient selection: hospitalized patient with SLE

 

Outcome: 30-day readmission (early)

 

Predictors of readmission within 30 days:

(1) increased Systemic Lupus International Collaborating Clinics/ACR Damage Index (SDI)

(2) low serum hemoglobin (anemia; use < 10.5 in the implementation)

(3) low serum albumin (use < 3.5 g/dL in implementation)

(4) government-sponsored insurance (Medicaid)

(5) history of prior immunization (OR for no prior immunosuppressives 0.61)

 

The odds ratios ranged from 0.66 to 1.93 so no single predictor dominated.

 

Readmission tended to occur in high risk patients with active disease.
