Description

Mottl et al reported factors predictive of progression to end-stage renal disease (ESRD) in a patient with diabetic nephropathy. These can help to identify a patient who benefit from more aggressive management. The authors are from the University of North Carolina.


Patient selection: diabetic nephropathy

 

Predictors from multivariate analysis:

(1) segmental sclerosis = SS (subdistribution HR 2.0)

(2) extracapillary hypercellularity = EXHC (subdistribution HR 2.2)

 

Extracapillary hypercellularity occurs at sites of rupture of dilated capillaries following mesangiolysis. Ut was defined as more than 2 layers of epithelial cells affecting > 10% of the surface of the glomerular tuft or Bowman's capsule and comprising > 10% cells.

 

Protective factors:

(1) older age in years (subdistribution HR 0.71)

(2) higher baseline GFR (subdistribution HR 0.75)

(3) mild mesangial expansion (subdistribution HR 0.64)
