Lung et al reported factors predictive of a poor outcome following ethylene glycol poisoning. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of California San Francisco.
Patient selection: acute ethylene glycol poisoning
Prolonged renal insufficiency was defined as the need for > 3 days after presentation.
Risk factors for a poor outcome (death or prolonged renal insufficiency):
(1) coma
(2) seizures
(3) intubation
(4) acidosis (mean initial arterial pH 7.03)
(5) time from presentation to antidote >= 6 hours (OR 3.3)
Protective factors:
(1) time from presentation to dialysis >= 6 hours (OR 0.36)
