Lasica et al reported predictors for early and late mortality for a patient with acute aortic dissection type B. These can help to identify a patient who may benefit from more aggressive management. The authors are from University Clinical Center Serbia in Belgrade and University of Illinois.
Patient selection: acute aortic dissection type B
Predictors of in-hospital mortality from multivariate analysis:
(1) uncontrolled hypertension (HR 20.7)
(2) maximum diameter of abdominal aorta > 4.75 cm (HR 6.3)
Predictors of late mortality from multivariate analysis:
(1) relapsing pain (HR 7.9)
(2) uncontrolled hypertension (HR 7.3)
(3) systolic blood pressure difference between the left and right upper extremities > 20 mm Hg (HR 5.3)
Surgical management is associated with a higher short-term mortality but better long-term survival.
