Description

Kyei et al identified factors associated with perioperative transfusion for men undergoing open prostatectomy for benign prostate hyperplasia (BPH) in Ghana. These factors can help to identify a patient who may benefit from interventions to reduce transfusion. The authors are from the University of Ghana.


Indications for prostatectomy:

(1) refractory retention of urine

(2) hematuria due to BPH

(3) lower urinary tract symptoms not amenable to other treatment

(4) BPH with bladder calculi

 

Predictors from multivariate analysis:

(1) preoperative hemoglobin < 11 g/dL

(2) duration of surgery > 90 minutes

 

Correcting anemia prior to surgery and reducing surgical duration may reduce the need for blood transfusion.

 

Additional factors from univariate analysis:

(1) emergency surgery

(2) general anesthesia (versus spinal or epidural)

(3) prostate weight > 100 grams

(4) resident as the operating surgeon
