Description

Kirkip et al identified factors associated with mortality in a patient with pulmonary sarcodiosis. These can help to identify a patient who may benefit from more aggressive management. The authors are the University of Firat (Elazig, Turkey) and the University of Cincinnati.

Independent predictors of mortality:

(1) older age at sarcoidosis diagnosis (>= 50 years)

(2) extensive fibrosis on high-resolution CT (> 20%)

(3) precapillary pulmonary hypertension (pulmonary artery mean pressure > 25 mm Hg with pulmonary artery occluding pressure < 15 mm Hg)

Additional factors associated with increased mortality in univariate analysis:

(1) Black race

(2) bad prognosis in Walsh model

(3) GAP stage 2 or 3

(4) Scadding stage 4 chest X-ray
