Kamiya et al reported factors associated with significant conditions requiring a clinical intervention. These can help to identify a patient who may need a more extensive workup. The authors are from Jichi Medical University and Fukushima Medical University in Japan.
Patient selection: adult with cervical lymphadenopathy
Predictors of a condition with some clinical significance:
(1) age >= 60 years (aOR 2.7)
(2) largest lymph node >= 2 cm in diameter (aOR 1.9)
(3) fixed lymph nodes (aOR 2.7)
(4) serum LDH >= 400 U/L (aOR 3.8)
Over 80% of patients with a serum LDH >= 400 U/L had infectious mononucleosis or necrotizing lymphadenitis.
Having a referral was also associated with a significant disorder, but this would be seen by a specialist or referral hospital (aOR 1.8). It may be an indirect measure for persistent lymphadenopathy.
