Description

Kalantri et al identified risk factors associated with in-hospital mortality following a snake bite in rural India. These can help to identify a patient who may benefit from more aggressive management. The authors are from Mahatma Gandhi Institute of Medical Sciences in Sevagram, India, and the University of California at Berkeley.


Patient selection: snake bite, admitted to hospital

 

Outcome: in-hospital mortality

 

Predictors of in-hospital mortality:

(1) vomiting

(2) neurotoxicity

(3) elevated serum creatinine on admission (50% mortality if serum creatinine > 2 mg/dL)

 

where:

• Viper envenomation is associated with renal failure which may be due to: hypovolemia,  intravascular hemolysis, DIC and/or venom-associated nephrotoxicity.

• Neurotoxicity was associated with krait or cobra bites.
