Kakkilaya et al reported findings predicting failure of continuous positive airway pressure (CPAP) in a preterm neonate. This can help to identify a patient who may require more aggressive management. The authors are from University of Texas Southwestern and Parkland Health and Hospital Systems in Dallas.
Patient selection: preterm neonate <= 29 weeks gestational age placed on CPAP for neonatal respiratory distress
Predictors of CPAP failure from multivariate analysis:
(1) FIO2 > 0.3 within 2 hours of birth (hours of life or HOL; OR 6.7)
(2) severe RDS on first chest X-ray (OR 21.5)
Additional factors that may be associated:
(1) a/A PO2 <= 0.22
The median time for CPAP failure was 5.9 hours.
