Description

Spontaneous fungal peritonitis is a rare but serious complication of cirrhosis. Huang et al identified factors associated with increased short-term mortality. The authors are from The First Affiliated Hospital of Zhejiang University in Hangzhou.

Patient selection: cirrhosis and spontaneous fungal peritonitis

Outcome: 15-day mortality

Fungal isolates: Candida species, Cryptococcus laurentii, Aspergillus, Trichosporon

Risk factors for spontaneous fungal peritonitis:

(1) recent antibiotic administration

Predictors of short-term mortality:

(1) hepatorenal syndrome

(2) serum total bilirubin >= 150 µmol/L

(3) SOFA score >= 8

(4) MELD >= 19

(5) WBC count >= 10 * 10^9/L

(6) serum creatinine >= 101 µmol/L (which may overlap with the hepatorenal syndrome)
