Description

Hsiao et al identified factors associated with mortality for a patient with necrotizing fasciitis. These can help to identify a patient who may benefit from more aggressive management. The authors are from Chang Gung Memorial Hospital and Kaohsiung Medical University in Taiwan.


 

Patient selection: necrotizing fasciitis > 18 years of age

 

Risk factors for mortality:

(1) infection with Vibrio or Aeromonas species

(2) hypotension (systolic blood pressure < 90 mm Hg)

(3) malignancy

(4) percent bands in the differential count >= 10%

 

The risk was greatest for malignancy with odds ratio 47.

 

The presence of hemorrhagic bullae was associated with a reduced risk (odds ratio 0.16).
