Description

Holman et al reported predictors of death or heart transplant in a patient with a mechanical circulatory support device. These can help to identify a patient who may require more aggressive management. The authors are from the University of Alabama, University of Pittsburgh, University of Michigan, Children’s Hospital Boston, University of Florida, University of Louisville, UNOS and NHLBI.

Patient selection: mechanical circulatory support device (recorded in INTERMACS)

Predictors of death or heart transplantation:

(1) cardiogenic shock (INTERMACS category 1)

(2) advanced age (age > 60 years of age)

(3) severe right heart failure (causing ascites and/or hyperbilirubinemia)

The risk increases with the number of predictors present.
