Description

Hocker et al identified factors impacting outcome and disability for a patient with refractory status epilepticus. These can help to identify a patient who may benefit from more aggressive management. The authors are from the Mayo Clinic in Rochester.


 

Patient selection: refractory status epilepticus

 

Outcome: modified Rankin scale (mRS), with functional decline defined as a change in the scale > 1 between admission and discharge

 

Predictors of mortality:

(1) prolonged mechanical ventilation

 

Predictors of poor functional outcome and disability:

(1) prolonged drug-induced coma

(2) cardiac arrhythmia requiring intervention

(3) pneumonia

(4) severe EEG suppression

 

where:

• Seizure control without suppression-burst or isoelectric EEG was associated with good functional outcome.
