Description

Higgins et al evaluated factors associated with death or new disability following an ICU admission with mechanical ventilation. These factors can help to identify a patient who may require more aggressive management (or palliative care). The authors are from multiple institutions in Australia and Brazil.


Patient selection: age 49-71 years (median 62); ICU admission on mechanical ventilation > 24 hours

 

Outcome: death or new disability by 6 months (59%)

 

Definition of disability: increase in WHODAS percentage >= 10% from baseline

 

Predictors of death or new disability:

(1) age (OR 1.02; will adjust based on lowest age of 44 years)

(2) APACHE III (OR 1.02; maximum score around 299)

(3) admission diagnosis (see below)

 

Admission diagnosis:

(1) surgical (OR 1)

(2) cardiac arrest (OR 4.1)

(3) sepsis (OR 2.4)

(4) trauma (OR 6.2)

(5) lung transplant (OR 0.2)

(6) other (OR 1.5)
