Description

Hagar et al identified predictors for paravalvular leak following transcatheter implantation of the aortic valve. Some degree of paravalvular leak is relatively common. The authors are from West China Hospital and Sichuan University in China and B & B Hospital in Nepal

Patient selection: transcatheter aortic valve implantation

Predictors of paravalvular leak:

(1) greater aortic valve calcification volume in cubic mm (mean 991.6 +/- 709.9, vs 604.4 +/- 479.1)

(2) larger annular dimensions (area and diameters)

(3) pre-replacement transvalvular peak velocity in meters per second (mean 5.15 +/- 0.75, vs 4.94 +/- 0.71)

There are significant overlaps between those with and without paravalvular leak.

Calcification locations associated with paravalvular leak:

(1) body of the leaflet

(2) cusps
