Description

Ghauri et al performed a systemic review of patients admitted to the intensive care unit (ICU) who required mechanical ventilation. They identified factors predictive of prolonged ventilation which may impact how the patient is managed. The authors are from Shifa International Hospital and Poonch Medical College in Islamabad, Pakistan.


Patient selection: ICU admission with mechanical ventilation

 

Definition of prolonged ventilation: > 24 hours (versus > 14 days)

 

Predictors of prolonged mechanical ventilation:

(1) renal dysfunction prior to ICU admission or acute kidney injury while intubated

(2) chronic obstructive pulmonary disease (COPD)

(3) elevated heart rate (> 110 beats/minute)

(4) APACHE III score > 68

 

Predictors in a patient undergoing cardiac surgery:

(1) repeated cardiac surgeries

(2) emergency cardiac surgery

(3) prolonged time on cardiopulmonary bypass (> 180 minutes)

(4) need for perioperative blood transfusions
