Description

Gershman et al identified predictors affecting mortality in patients with renal cell carcinoma. This can help to identify patients who may benefit from more aggressive or novel therapy. The authors are from Rhode Island Hospital, Miriam Hospital, University of Illinois, and the Mayo Clinic Rochester.

Patient selection: renal cell carcinoma pN1M0

Outcome: all-cause and cancer-specific mortality

Parameters:

(1) ECOG performance status

(2) sarcomatoid differentiation

(3) coagulative tumor necrosis

where:

• The odds ratios range from 2.2 to 2.5 for these risk factors for both all-cause and cancer-specific mortality.

total score =

= SUM(points for all 3 parameters)

Interpretation:

• minimum score: 0

• maximum score: 3

• The higher the score the greater the risk for a poor outcome.

Performance:

• Surgical selection bias was noted.
