Description

Flythe et al reported factors predictive of hospital readmission for a patient receiving maintenance hemodialysis. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of North Carolina.


Patient selection: adult on maintenance hemodialysis

 

Outcome: 30-day readmission

 

Predictors of readmission (model 2 using discharge data):

(1) malignancy (odds ratio 2.1)

(2) >=3 hospital admissions in the prior year (odds ratio 2.0)

(3) >= 10 outpatient medications on admission (odds ratio 1.7)

(4) catheter vascular access (odds ratio 1.8)

(5) outpatient dialysis at a non-university affiliated dialysis facility (odds ratio 3.6)

(6) intradialytic hypotension (odds ratio 3.1)

(7) discharge over the weekend (odds ratio 1.8)

(8) serum albumin < 3.3 g/dL (odds ratio 4.3, any time during admission)

 

Reducing the number of medications between admission and discharge is associated with a lower readmission rate (odds ratio 0.20).
