Description

Ferguson and Macdonald identified factors associated with cerebral infarction following aneurysmal subarachnoid hemorrhage (SAH). A patient with a cerebral infarct secondary to a SAH is more likely to have a poor outcome. The authors are from the University of Chicago.


 

Patient selection: recent aneurismal subarachnoid hemorrhage

 

Predictors for cerebral infarction on multivariate analysis:

(1) older age (>= 60 years)

(2) worse neurological grade on admission (World Federation of Neurological Surgeon = WFNS Grade 3, 4 or 5)

(3) history of hypertension

(4) history of diabetes mellitus

(5) greater diameter of the aneurysm (>= 13 mm)

(6) hypertension induced prophylactically or therapeutically

(7) temperature on day 8 after the SAH > 38°C

(8) presence of symptomatic vasospasm

 

where:

• The rise in temperature on day 8 may be a sign of vasospasm.

 

Avoiding vasospasm may be one way to improve outcome.
