Dutta et al evaluated patients who developed acute kidney injury (AKI) following cardiac arrest. Acute kidney injury following cardiac arrest is associated with a worse prognosis. The authors are from Wake Forest University and University of Massachusetts.
Patient selection: cardiac arrest survivor
Exclusions: anuria, end-stage renal disease, CKD stage 5
Predictors for acute kidney injury:
(1) higher serum creatinine on admission (?? existing CKD)
(2) use of vasopressors
Predictors for post discharge hemodialysis:
(1) use of vasopressors
Risk associated with acute kidney injury (AKI 1 or higher):
(1) increased mortality (OR 1.4)
(2) poor neurological outcome (cerebral performance category > 2; HR 2.3)
(3) disability (modified Rankin scale > 3; OR 2.2)
Post discharge dialysis was associated with:
(1) receipt of vasopressors
(2) increased risk of mortality (OR 2.6)
