Description

Drewry et al identified several risk factors associated with mortality in patients with sepsis. These can help to identify a patient who may benefit from more aggressive management. The authors re from Washington University and Barnes-Jewish Hospital in St. Louis.


Patient selection: sepsis

 

Outcome: 28-day mortality

 

Predictors of mortality from multivariate analysis:

(1) older age

(2) higher APACHE II score on admission (after removal of age component, will use > 20 in implementation)

(3) liver disease

(4) longer time to appropriate antibiotic therapy

(5) presence of absolute lymphocytopenia on day 4 after sepsis diagnosis. Severe lymphocytopenia (<= 600 per µL) had a greater effect than moderate thrombocytopenia (700 to 1,000 per µL) versus no lymphocytopenia.
