Description

De Pascale et al identified factors predictive of 28-day mortality for a critically-ill patient with a complicated intra-abdominal infection. The authors are from Universita Cattolica del Sacro Cuore in Rome.


Patient selection: critically-ill patient with a complicated intra-abdominal infection (infection extending beyond the affected hollow viscus with localized or diffuse peritonitis)

 

Outcome: 28-day mortality

 

Predictors for 28-day mortality from multivariate analysis:

(1) initial inadequate antimicrobial therapy (OR 11.4)

(2) inadequate source control (OR 4.1)

 

Factors that may impact the predictors:

(1) septic shock

(2) multi-organ failure, including ARDS, with higher SAPSII and SOFA scores

(3) multi-drug resistant organisms

(4) age of the patient and comorbidities
