Description

Dalziel et al reported independent predictors for severe H1N1 infections in pediatric patients. These can help identify patients who may benefit from preventive measures or more aggressive management. The authors are from the Pediatric Emergency Research Network (PERN) which includes researchers from around the world.


 

Patient selection: pediatric patient with H1N1 influenza

 

Severe outcomes: admission to the ICU (need for inotropic support and/or mechanical ventilation), death

 

Risk factors for severe influenza:

(1) history of chronic lung disease

(2) history of developmental delay and/or cerebral palsy

(3) clinical chest retractions

(4) clinical dehydration

(5) clinical need for supplemental oxygen

(6) clinical tachycardia for age

 

A pediatric presenting with one or more of these findings is at increased risk for a severe outcome. A patient with one or both of the historical risk factors may be a candidate for prophylaxis.
